Who:<\/span>  <\/strong>Hospice supports patients and their families in the communities of Benton, Lincoln, Linn, Polk and Tillamook counties.<\/p>\n\n\n\n

What:<\/span>  <\/strong>Hospice is a philosophy of care for people of all ages in the last stages of terminal illness.<\/p>\n\n\n\n

Where:<\/span><\/strong> Patients may stay wherever they call home until their final days. In addition, the Samaritan Evergreen Hospice House offers a \u201chome\u201d where patients can stay. Located in Albany, it is one of only three inpatient hospice facilities in Oregon.<\/p>\n\n\n\n

When:<\/span> <\/strong> Comforting care during the final days of the patient\u2019s life and support for family members during and after their passing.<\/p>\n\n\n\n

Why:<\/span>  <\/strong>With a focus on patient comfort, hospice provides resources to manage symptoms and give respite to caregivers. The hospice care team provides seamless coordination with other medical providers.<\/p>\n\n\n\n
