Use this online form<\/a> to request your medical records from a past stay at one of our Samaritan hospitals or visits to any of our Samaritan medical clinics throughout Benton, Lincoln and Linn counties. Please allow 30 days<\/strong> to process requests for medical records. Depending on the purpose of your request, standard fees may apply for copies of your records.<\/p>\n\n\n\n
