Please schedule your appointment at least one month before planned departure. Once appointment is scheduled pre-travel forms will be loaded into <a href=\"https:\/\/mychart.samhealth.org\/MyChart\/Authentication\/Login?\" target=\"_blank\" rel=\"noreferrer noopener\">MyChart<\/a>. You can expedite your appointment by digitally filling out the MyChart forms. If not using the digital option please bring the printed pre-travel forms to your appointment.<\/p>\n\n\n\n<p>At your pre-travel appointment, we will review your itinerary and discuss ways to prevent food and water-borne illnesses such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hepatitis A.<\/li>\n\n\n\n<li>Typhoid fever.<\/li>\n\n\n\n<li>Traveler\u2019s diarrhea.<\/li>\n<\/ul>\n\n\n\n<p>We will also discuss ways to avoid insect-borne illnesses such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Malaria.<\/li>\n\n\n\n<li>Dengue fever.<\/li>\n\n\n\n<li>Yellow fever.<\/li>\n\n\n\n<li>Japanese encephalitis.<\/li>\n<\/ul>\n\n\n\n<p>We will also cover any other endemic bacterial, parasitic or viral infectious unique to your itinerary and administer appropriate vaccinations, including yellow fever.
